Getting Ready for Your General Surgery Appointment
Please arrive at least 15 minutes before your scheduled appointment time and be sure to have the following items available for reference:
- Health insurance card
- Photo ID
- Completed patient forms
- Immunization records (if available)
- A current list of your medications
